- Physicals are
requested from all new students and
10th grade students.
- Immunization
requirements, per NYS, are mandatory for all students.
- Dental certificates
are requested with physicals.
- Medication
orders must be renewed each year.
If your daughter is taking medication of any kind at school and that
includes over-the-counter medication, we should have a Doctor's order on
file. It's easy to obtain an order; your
Doctor can fax it to 825-0976 Attention:
K. Burns, RN.
- Flu shots are
available in the community. Please
consider receiving a flu shot or if shots are not your thing, try flu mist.
